While linking, I found the following page,

where it appears an intended surname correction for the domestic must have been incorrectly populated to the rest of the family (transcribed: Margaret Nicholls/proofed: Cheryl Martin). As no proofing notes remain, this is my best guess.

Could we have someone change the surname on lines 32 to 38 back to Forrest from Okasaki (line 39).

Thanks Gord,

When I didn't see any proofing notes I began to get worried. I just hope that no other errors like this exist (i.e. "post-proofing")

Unfortunately, there may be any number of these, just because the Surname Correction function can't handle the strays, like domestics. The transcriber who accepts the correction instead of editing the line will set this up every time.
